The Animal Bite Treatment Center Management Information System (ABTC MIS) is a proprietary information system maintained for the exclusive use of PROVIDENCE ANIMAL BITE & MEDICAL CLINIC. All forms, reports, databases, workflows, records, and system components are intended for internal clinic operations only.
Unauthorized copying, modification, redistribution, commercial use, or deployment of this system without written authorization from clinic management is prohibited.

This system has been designed to support the documentation, monitoring, reporting, and operational requirements of an Animal Bite Treatment Center in accordance with applicable Department of Health (DOH) policies, standards, and guidelines.

Users are responsible for protecting their login credentials and ensuring that patient information is handled in accordance with clinic policies and applicable privacy regulations.
Medical Documentation
Records generated by this system, including Patient Information Sheets (PIS), Exposure Records, Vaccine Records, Clerking Sheets, Progress Notes, Referrals, and Invoices, form part of the clinic's official medical documentation and shall be maintained and archived according to clinic record retention policies.
Limitation of Liability
While reasonable efforts are made to maintain system accuracy, reliability, and security, PROVIDENCE ANIMAL BITE & MEDICAL CLINIC shall not be liable for losses arising from unauthorized access, hardware failure, internet interruptions, power outages, accidental deletion, or failure to maintain proper backups.
Regular database backup and system maintenance procedures are strongly recommended.
